All required forms must be completed 100% and submitted before a student participates in any try-out, practice, athletic class, open gym, open weight room, athletic competition, or travels with an athletic team for any purpose. This is to include the athlete and any extracurricular assistance (i.e.: student managers, video, equipment, statisticians, student athletic trainers).

The student is required to use the Pre-participation - Physical Examination Form located under the Download and Print tab. NO OTHER Physical Examination Form can be accepted as per the U.I.L. A new physical exam must be given prior to each school calendar year.
